Steps for Renewing Your Policy
If you purchase your MPLI policy directly from KAMMCO, follow the steps below to renew your policy for the upcoming year.
Step 1: Select and complete your renewal application.
Required.

Step 2: Download and complete the appropriate payment forms.
Required.
Click the button below to download the payment forms packet. Then, complete the appropriate forms for your payment situation.

Step 3: Optional Premium Financing Option form.
Optional.
Download and complete the Finance Premium Acceptance Form if you want to use third-party financing from Fidelity State Bank.
Do not send this form to KAMMCO. Follow the instructions in the form to send it directly to Fidelity State Bank.

Step 4: Determine if you want higher limits for Cyber Coverage.
Optional.
Every KAMMCO policy comes with embedded cyber coverage. CLICK HERE for more information about cyber coverage.
Click the button below to download and complete the Cyber Breach Response Application if:
- You previously opted to have higher limits on cyber coverage and would like to continue with that option in 2025 or
- You want to increase your cyber coverage limits beyond your embedded cyber coverage limits.

Step 5: Submit Your Renewal Materials for Processing
Important. Please Read.
- Submit all materials together at the same time.
- We cannot process your renewal until we receive all your renewal materials. Submitting materials one at a time will delay processing.
- To process a renewal for a group policy, we must receive the renewal forms for each individual on the policy before proceeding.
Deadline for Submission
For policies that renew on January 1, 2025, the deadline to submit renewal materials is October 11, 2024.

2. (Required) Election of Payment Form
3. (Optional) Additional payment forms, as needed.
3. (Optional) Cyber Breach Response Application
